To submit your correspondent loans to us, please have it registered and use the below form as your cover page.  

Running AUS (DO) 

To Run AUS through us, you must request sponsorship through Fannie Mae. When searching for Lenders, search for Open Mortgage, LLC. You will be able to run AUS through your LOS or in the file

Checklist for Minimum items to submit 

Submission Cover Sheet – Fully Completed
□ Applicable State Disclosures
□ Initial 1003 – Fully Completed, including HMDA fields
✓ LO & Borrower Signed, initials where applicable
✓ Application Date: (Purchase – Date Property Address was obtained: Refinance – Date should be on or before credit report date)
□ LE – Prepared date within 3 days of 1003 – No blank dates
□ Itemization of Fees/Fees Worksheet to match LE
□ Intent to Proceed
□ Acknowledgment of Receipt of LE

□ Itemization of Amount Financed
□ HUD Counseling Disclosure
□ Servicing Disclosure – fully completed
□ Appraisal Air Disclosure
□ ECOA Appraisal Disclosure
□ Equal Credit Opportunity Act Disclosure
□ Service Provider Form, if State required or if items listed in Section 6 of the GFE
□ Notice to Home Loan Applicant Credit Score Info Disclosure (usually final pages of credit report)– signed by borrower OR provide Proof of Delivery of disclosure to borrower
□ Risk Based Pricing/Your Credit Score and the Price You Pay for Credit - signed and dated by borrower OR Proof of Delivery
□ Disclosure Notices
□ Undisclosed Debt Acknowledgment Form
□ Copy of Driver’s License – must be legible
□ 4506T – fully completed
□ Occupancy Disclosure
Mortgage Broker Disclosure – as applicable
□ Privacy Act Disclosure - Page 2, Section 1 and 3 needs to be completed
□ Flood Disaster Protection Act of 1973
□ E-Sign Certification
□ Borrower’s Certification & Authorization
□ Fair Credit Reporting Act
□ Patriot Act Disclosure
□ Patriot Act Customer Identification Form
□ Affiliated Business Arrangement Disclosure – if none exist, complete top of form, write NONE and sign
□ Proof of Delivery of HUD Settlement Book
□ Program Specific Disclosures (i.e.: USDA 1980-21, FHA 92900A, etc.)

  • 30 days of paystubs 
  • 2 months bank statements (Or VOD on Conventional) 
  • 2 years tax returns (If self employed or commissioned) 
  • 2 years w-2's 
  • Tri merge credit report (Expires within 100 days of pull) 
  • VOR If applicable 
  • VOE
